* Country View Senior Living Community offers an ala carte program where the family
and/or tenant can pick and choose what services they would like to have. The following
is a list of the services that we currently offer, we can always add more to meet
the needs of our tenants.
OPTIONAL PERSONAL/HOME CARE SERVICES
NOT COVERED IN THE COMMUNITY’S RENT
(MONTHLY CHARGES)
The Community, or a provider designated by the Community,
can provide you with optional personal/home care services as set forth below upon
request. However, You have the right to select any outside provider (licensed) you
desire to obtain needed supplies or services. If you choose an outside provider,
you pay the provider directly.
DESCRIPTION OF HOME CARE SERVICES
Incidental Nursing:
Assistance to tenants with anything other than services covered by their monthly
rent and/or service agreement. Examples of these services may include extra
linen
change, answering calls, and/or short term physician orders such as checking
blood
pressure, weights, foot soaks, etc. Billed in 15 minute increments, and are
performed by either HHA or licensed nurse, depending on service.
Bathing Assistance I:
Assistance with bathing/showering in tenant’s apartment or whirlpool. Does not
include any other assistance.
Bathing Assistance II:
Assistance with bathing/showering in tenant’s apartment or whirlpool. Includes
getting in/out of shower, assist with washing , rinsing, drying, lotion, nail
care,
shampooing hair as requested.
Dressing/Grooming Assistance AM or PM:
Includes assistance with putting on or removing clothing, including TED
stockings.
Also includes assistance with grooming needs such as combing hair, applying
deodorant, shaving, oral care, washing face.
Dressing /Grooming Assistance AM/PM:
Includes same services as Dressing/Grooming Assistance AM or PM, but the service
is provided both morning and evening.
Assistance with TED Hose or Compression Stockings:
Includes assistance with putting on stockings in the AM, removing them at night.
Service also includes lotioning legs, assistance with shoes, and washing
stockings at
night.
Assistance with Toileting or Incontinence:
Assistance on/off toilet or commode with limited transfer assistance, changing
incontinent products, providing hygiene after changes, changing clothing as
needed,
emptying commode, monitoring skin condition. Billing is in accordance with the
amount of time and number of times assistance is needed up to 6x per 24 hours.
Assistance with Transfers:
Assistance getting in and out of chair, on and off toilet, transferring from
chair to
bed with assist of one. Transfers may not exceed 5x/day.
Safety Checks:
Includes staff members checking on tenant to ensure their safety/well-being.
This
service may be provided from hourly to daily. Billing is in accordance with
frequency of checks required.
Escort Assistance:
Physical/standby assistance to destinations within the facility for those
tenants with
mobility limitations. Limited assistance with transfers. Includes escort to
meals and
activities. Service does not include redirection for confused tenants.
Reminders/Redirection:
This service is provided to those tenants who, due to confusion, forgetfulness,
have
difficulty finding specific locations, or need frequent reminders. It includes
reminders of mealtimes, physical assistance to dining room or other areas of the
facility, including the tenant’s apartment.
Medication Assistance:
Includes administration of medications as ordered by the physician, ordering
medications from the pharmacy, communication with the physician in relation
to changes in medications, and central storage of medications. Administration
includes oral medications, eye drops, nasal sprays, inhalers, and topical skin
lotions/ointments. May also include insulin administration. Billing is in
accordance
with the number of medications and the number of medication passes required.
Medication Set-up:
Medication set-up 2x month or weekly in medication containers by licensed nurse.
Tenant administers medications independently, or medications are administered
to tenant by staff. Includes reordering of medications, assessing medication
compliance, and correspondence with the physician.
Medication Changes:
Includes transcribing new orders, notifying pharmacy of new orders,
Instruction staff on usage and side effects, and in some instances, training
staff if
specific orders are given.
Oxygen Assistance:
Ordering O2 as needed, assistance with management of the equipment, ordering
supplies as needed, washing filter weekly, changing tubing monthly, filling
portable oxygen as needed, monitoring of liters ordered, consulting with
physician
regarding problems and compliance. Also includes cost of running concentrator
24/7.
Oxygen Concentrator Usage:
Covers the additional cost of electricity needed for continuous use of oxygen
equipment.
Glucose Monitoring:
Blood sugar testing as ordered by physician, contacting physician with
abnormalities, ordering supplies as needed, documentation of readings.
Billing in accordance with frequency of monitoring.
Weights:
On-going weight monitoring as ordered by physician, contacting physician with
significant changes, documentation of weights. Billing in accordance with
frequency.
Blood Pressure Checks:
On-going blood pressure monitoring as ordered by physician, contacting physician
with abnormalities, documentation of readings. Billing in accordance with
frequency.
Eye Drop Administration:
This service is for pre and post-cataract surgery. It covers the numerous times
eye drops are administered for the time period designated by the physician. This
is
a short-term charge.
Skin Care:
This service is dependent on the type of skin care indicated. It may include
applying lotion daily or other skin care as requested by the tenant. Billing is
in
accordance with frequency of service.
